Greetings,

Are you a resident of Ethiopia, and a fluent reader of English, Amharic, or Oromo? Have you watched the music video “Lib Yaleh” (If you have heart), or heard the song? If so, please consider participating in this survey.

Your participation will help us to understand the extent of the song’s impact and diffusion. It will also help us to create more effective musical interventions in the future, in Ethiopia and elsewhere. Participating may help improve global health. We do not foresee any risks. There is no cost to participate.

Your participation in this survey is completely voluntary and—if you wish—completely anonymous. Emails will not be collected unless you so authorize. Results will always be reported in aggregate, or anonymously by case; regardless of whether or not you elect to remain anonymous, data you provide will never be publicly linked to your name or other identifying characteristics in any research papers or presentations resulting from this research. Personal information will never be shared with other individuals or organizations. Completed survey data will be kept in a secure location on the University of Alberta campus; only project researchers will have access.

Providing your name and contact information will enable follow-up interviews, as well as acknowledgement of your research contribution, if you so choose. However anonymity will not decrease the value of your responses. Your identifying information name, email (if collected) will be securely stored for researcher access only. The information you provide will be used only for the indicated purposes in conformity with the Alberta Freedom of Information and Protection of Privacy (FOIPP) act.

You may choose to omit any questions you do not wish to answer. However some questions may require an answer in order for the survey to be completed. If you nevertheless do not wish to answer the question, you may close your browser and withdraw from the survey.

You may end your participation at any time by simply closing your browser. Information will not be transmitted until you click on the send button which will appear at the conclusion of the survey. At that point your answers cannot be withdrawn.

A final report, when available, will be emailed to you upon request. The information you provide will be used only for the indicated purposes in conformity with the Alberta Freedom of Information and Protection of Privacy (FOIPP) act.

The plan for this study has been reviewed by a Research Ethics Board at the University of Alberta. If you have questions about your rights or how research should be conducted, you can call +1 (780) 492-2615. This office is independent of the researchers.
