General instructions (revised and simplified)
1. First, right click on "VMCTM prototype", selecting Open Link in New Window, to open a new browser window containing the prototype.
2. Next, click on "Feedback" to open the consent form and survey; follow its directions to complete both.
3. Save this document to your desktop, and email it to us by clicking here, then attaching the document.
